NOTIS PENGECUALIAN KELAS KO KURIKULUM

 

PERMOHONAN PENGECUALIAN KELAS YANG TIDAK AKAN DIPERTIMBANGKAN

Berikut adalah senarai aktiviti yang tidak akan dipertimbangkan untuk permohonan pengecualian kelas kursus kokurikulum:

  1. Melancong (termasuk mengerjakan umrah)
  2. Aktiviti keagamaan
  3. Menghadiri upacara kenduri saudara mara termasuk adik beradik
  4. Menghadiri majlis konvokesyen saudara mara termasuk adik beradik
  5. Mengulangkaji pelajaran
  6. Sebarang aktiviti atau menghadiri upacara yang tidak sesuai untuk dipertimbangkan oleh pihak pengurusan pusat rancangan kokurikulum

Nota:

Permohonan pengecualian kelas perlu disertai dengan borang permohonan (KK/PTJPK/PKKK/BPMAK-01-V2) yang telah diluluskan oleh timbalan pengarah / pengerusi rancangan sebelum cuti diambil.

APPLICATION FOR CLASS EXEMPTION THAT WILL NOT BE CONSIDERED

The following is a list of activities that will not be considered for the co-curriculum class exemption application:

  1. Travelling (including umrah)
  2. Attending religious activity
  3. Attending wedding ceremony of relatives including siblings
  4. Attending convocation ceremony of relatives including siblings
  5. Reviewing the lesson
  6. Attending any activity or event that is inappropriate for consideration by the Centre of Co-curriculum Programme

Note:

Class exemption applications must be attached with the application form (KK / PTJPK / PKKK / BPMAK-01-V2) which has been approved by the deputy director / chairman of the centre before the leave is taken

PENGUMUMAN PENDAFTARAN KURSUS RANCANGAN KOKURIKULUM

ANNOUNCEMENT

CO-CURRICULUM COURSE REGISTRATION

SEMESTER 1, ACADEMIC SESSION 2019/2020

 

Sukacita dimaklumkan bahawa aktiviti pendaftaran secara dalam talian bagi semua kursus Rancangan Kokurikulum Kampus Kesihatan bagi Semester 1, Sidang Akademik 2019/2020 akan dibuka seperti maklumat berikut:
Kindly be informed that the online registration for all co-curriculum courses in Health Campus for Semester 2, Academic Session 2019 / 2020 will be made available as per details below:

  1. Tarikh pendaftaran: 10 – 14 September 2019 (Bermula: 2:30pm)

 Date of registration: 10  – 14 September 2019(Starting form: 2:30 pm)

 

  1. Tarikh pendaftaran DENGAN kuota pusat pengajian: 10 – 12 September 2019 (Sehingga jam 5:00pm)

 Date of registration WITH school quota: 10 – 12 September 2019 (Not later than 5:00pm)

 

  1. Tarikh pendaftaran TANPA kuota pusat pengajian: 12 Sept 2019 (Bermula: 5:01pm) – 14 Sept. 2019

Date of registration WITHOUT school quota: 12 September 2019 (Starting from: 5:01pm) – 14 September 2019

 

  1. Tarikh gugur / tukar kursus atas talian (e-daftar): 15 - 26 September 2019 (Tanpa penalti)

Change / drop of courses via online (e-daftar): 15 – 26 September 2019 (No penalty)

 

  1. Tarikh gugur kursus secara manual: 27 – 30 September 2019 (Dengan penalti [RM50.00])

Drop of course manual: 27 – 30 September 2019 (Penalty [RM50.00])

  1. Sila gunakan ID login (user name & password) untuk portal Campus Online.

Please use your  login ID (username & password) for Campus Online portal.

  1. Senarai kursus serta maklumat berkaitan penawaran kursus (jadual, maklumat penilaian kursus dsb.) boleh dirujuk di laman web Pusat Rancangan Kokurikulum Kampus Kesihatan di URL: kck.usm.my/pkkk / Facebook: (https://www.facebook.com/Hebahan-Pusat-Rancangan-Kokurikulum-Kampus-Kesihatan-2051918681501934/).

The list of courses and related information (course schedules, assessments, registration requirement and etc.) can be referred through   Centre for Co-curriculum Programme website at www.kck.usm.my/pkkk / Facebook:( https://www.facebook.com/Hebahan-Pusat-Rancangan-Kokurikulum-Kampus-Kesihatan-2051918681501934/).

  1. Untuk aktiviti kelas pertama, sila rujuk jadual aktiviti mengikut kumpulan kursus masing-masing.

For the first class activity, please refer to the activity schedule according to the respective course groups.

  1. Sebarang MASALAHakses atau jika anda TERLUPA ID login Campus Online atau anda MASIH BELUM MEMILIKI ID login Campus Online, sila dapatkan bantuan PPKT di talian: 09-7673000  (ext:1134). Waktu operasi: 8:30am – 4:30pm (Hari bekerja).   

If you encounter any registration problems such as FORGOTTEN or DO NOT HAVE any Campus Online Login ID, please get assistance from PPKT at: 09-767 3000 (ext:1134). Operation hours: 8:30am – 4:30pm (Working hours).
